Requirements and Eligibility & Referral Sources:
18 years of age or older and have a medical or mental condition that requires treatment or rehabilitation services prescribed by physician or psychiatrist.
Physical and or mental impairment that handicaps activities of daily living but not so serious as to require 24-hour institutional care.
Reasonable expectation that preventive services will maintain or improve the present level of functioning and/or prevent further decompression or institutionalization.
Debilitating disorder such as:
Developmentally Disabled
Parkinson's Disease
Depression
Isolation
Stroke
Mental Impairments
Early Alzheimer's Disease
Elderly with physical and/or mental disability
